Enrollment dates for all sites:

July 17th and 18th - 8:00am to 3:00pm

Enrollment Documentation needed to attend school:

            IF ENROLLMENT IS A NEW STUDENT TRANSFER, REFER TO THE SITE PRINCIPAL

Ø Live in the school district:  Provide Documentation if a new student or registering in a different site.

Ø Birth Certificate

Ø Social Security Card if available

Ø CDIB if applicable and need to fill out Title VII Form

Ø Immunization records and OSIIS release form

Ø Papers from previous school; i.e. transcripts, report cards, withdrawal papers if applicable

Ø Let APS know if the student has IEP or any other special needs that need to be addressed
